LifeAct
We are helping LifeAct reach and enroll more schools in their life-saving depression awareness and suicide prevention education program for youths in grades six through 12. We created a website that makes it easy for school administrators to learn about and sign-up for LifeAct's programs. The site also provides an easy-to-use resource library for students as well as educators. In addition to an intuitive content management system to keep their site content up to date, our custom web tools allow LifeAct staff to create web-based presentations for use in schools with unique URLs and two-way poll participation.

Our custom Presentation Builder allows LifeAct to quickly create engaging web-based presentations. The tool automatically creates a unique per-session URL that collects polling and quiz data that participants send using their mobile devices.
Our thoughtful information strategy and website architecture for the LifeAct site helps move visitors through predetermined pathways to desired actions such as submitting a contact inquiry or downloading a resource.
We strategized and designed a map to help credentialize and build trust in LifeActs' programs by showing at a glance LifeAct's presence throughout the region. Our CMS provides custom tools that allow LifeAct to easily add pins to the map as their reach grows, too.
Our mission is to connect with people and give them ways to understand how to recognize depression and prevent suicide. To do that efficiently and reach more people, FORM reinvented our online presence. Our visual design for the site creates an overall welcoming tone that inspires trust. The power navigation provides one-click access to the site's most critical content and photography helps express an atmosphere of positivity and hope.
