FORM co-founder Jeff Kahooilihala has illustrated the cover art for Cartoon Network's 12 oz. Mouse Season One DVD. The illustration depicts characters from the quirky Adult Swim cult series in an homage to Davinci's Last Supper. The DVD also features 27 of Jeff's other 12 oz. Mouse pieces, including the full cast and accouterment sculpted in clay.

The series, created by Matt Maiellaro (Space Ghost, AquaTeen Hunger Force), revolves around a cab-driving mouse who robs banks with a chinchilla.
